In order to submit a claim, please fill out the claim form and attach all relevant
documents. You will need to submit the following information to
claims@go.miwaylife.co.za
1. A fully completed MiWayLife claim form.

2. A certified copy of the Death Certificate (if death)
3. A fully completed DHA-1663 form (if death)
4. Certified copies of the ID of the insured life or of all the beneficiaries
5. Three months bank statements for the beneficiary(ies)
